It's basically a list of your wishes for your labour and birth.
If there's anything you reall want, pain med wise or a water birth. If there's anyone you definitely don't want in the room. Who you want to cut the cord. Whether you want artificial stimulant to birth the placenta (usually a needle in your leg). Whether you want baby to room in, and if you are bf or ff. Whether you want baby to have oral vitamin k or a vitamin k shot or whether you want them to have vitamin k administered at all, etc etc.

I'll give you mine for example:

(minus the basic info like name, due date, partner's name, etc.)

I would prefer not to be induced. I would prefer the following methods if necessary:
- stripping membranes
- prostaglandin gel

I would like the following during labor:
- mobility
- food & water

I would prefer NO pain medication.
I would like no limits on laboring and prefer labor not be augmented (or induced) unless medically necessary.
I would like to wait to push until I feel the urge, even if I am fully dialated.
I would like to avoid c-section. If necessary, I would like my partner present.
I would like to have my partner cut the cord.
I would prefer to breastfeed. NO PACIFIERS OR BOTTLES!
I would like to have baby's circumcision done after discharge.

oooh thanks girls, this has defo helped!
i went for my growth scan today + they did mention that i needed to start putting it together but didn't say what exactly!

but bloody hell, you're brave! no pain medication? :O i'd love to do it with no pain medication but i'm such a wimp, i wouldn't last 2 seconds!

haha. I'm probably crazy for not wanting pain medication.. I just wanna be in and out of the hospital as soon as possible.. plus I want complete mobility.. with an epidural you have to stay in the bed.. screw that!
