Blood work for fertility specialist

Hi ladies, I finally get to see a fertility specialist in November. I have to go for my blood testing on days 2-5 of this menstrual cycle. I don't see anywhere on the specimen collection requisition form saying if I have to fast or not. Does anyone know?

Here's a list of what they are testing for:
Prolactin
Thyroid stimulating hormone
Follicle stimulating hormone
Luteinizing hormone
Estradiol
HIV antibody
Hepatitis B surface antigen
Hepatitis c antibiotics
HTLV I & II
Rubella lgG
Type and Screen
CBC
Syphilis TP antibody

U dont have to fast actually my FS told me to eat cuz they took like 18 vials of Blood!!
