yes you do need to. You are supposed to go to the loo no later than an hour before your scan and then try to drink at least a pint of liquid and hold it in.
it should say weather you should or shouldnt on your hospital letter it said nothing on mine so im not going to i think its more if you have an early scan as there is not much water for the ultrasound waves to travel through therefore its hard to get a pic
At 12 weeks your bladder is still in front of your uterus though and could block the view if you don't have enough fluid. At 20 weeks you don't need a full bladder as the uterus has gone higher up.
If they can't see properly they'll just do an internal scan anyway so don't panic, but follow the instructions on your letter.
I drank loads for my nuchal scan last time and couldnt hold it in any longer so went for a wee about 10 mins before the scan. I was worried i would mess the scan up but the woman told me they have to recommend a full bladder to everyone but that its more for overweight people (no idea why though) and that just a normal sized drink about 30 mins before should be sufficient for most people. I have no idea if she was just a bit weird or if this is true but im certainly not gonna force myself to drink loads this time!
