When the lines are equal, that's a pos test, so you didn't miss it. Lets say, you tested twice and they weren't as dark as the control but getting darker, then the next day, you tested and it was getting lighter, you then would have missed seeing the actual surge on the OPK but mostly likely it occurred between tests. That doesn't mean you missed O since O doesn't occur for 12-36hr later, sometimes even 48hrs later (average being 24hrs).
