Who can sign the form for healthy start vouchers??

The midwife gave me a form in my booking in pack. I know I would be entitled as I already get them or my DS. I won't see my midwife again now til 16 weeks but I have my 12 week scan on Monday so will there be anyone that would sign it for me there?

Needs to be a health professional. Usually a midwife or a health visitor.

I phoned my doctors up and they got the midwife to sign it for me.
