So my 9 week old (4 weeks early....so kinda 5 week size/development) has 2 settings, sleep or screaming.

We are on gaviscon atm as my other son was like this too and they prescribed IT, but apart from a little less puke and thicker nappies I see nothing different in him. I thought it may give him some sort of pain relief but he seems to cry all the time.

But......at night he is an amazing sleeper. Last feed and straight to bed at 9:30 and will wake him about 4 for feed then will sleep till 8am! No problems, no crying, just drifts off.

Wouldn't the reflux bother him no matter what time?

What are you doing to comfort him during the day? I found when nothing else worked, we'd put our daughter in the wrap and go outside and walk around the garden. Sometimes dance around the garden. 9 times out of 10 it worked. Keeping her upright and close to me all the time seemed to settle her easier and the fresh air made a huge difference too when that didn't work by itself. I would agree if he's sleeping that well and comfortable at night, it probably isn't the reflux that's causing it. I found when nothing else worked getting out of the house in some way helped.
 
My little girl has silent reflux and is often uncomfortable throughout the day after some feeds. I find putting her in the sling really helps as it keeps her upright. She settles in it really well even if she's in the middle of an episode.

I've also read that reflux is intermittent and doesn't necessarily happen after every feed. My DD is sometimes ok, sometimes not.
 
Thing is with him he isn't happy any time he is awake, at all. Even if I have him in the sling he either screams or is asleep, no Middle ground. I just don't know what to do.

Does he take a dummy? Dummies are good for reflux babies as the excess saliva helps neutralise the acid.

You could try taking him back to the doctors as the gaviscon isn't working. It didn't work for my little girl and she is now on ranitidine which can make a difference. It hasn't made much of a difference for DD and we are currently trying a cow's milk free formula in case the reflux is caused by a milk intolerance whilst waiting for a hospital appointment. I think it's worth considering. If he's screaming so much during the day, he might be sleeping so well at night because he's exhausted.
