hi ladies... so this is my 5th pregnancy
1st - early mc
2nd - early mc
3rd - born at 36+6
4th - premature twins born 27+3 :angel::angel:

im currently 14+4 weeks and was with my consultant yesterday and he strongly urged me to hav the cervical cerclage..
he measured my cervix which is 7.8cms which he said is really good so im assuming i do not hav IC but he still wants me to the stitch in place to try and prevent me going into preterm labour again

has anybody else had this procedure without having IC but for preterm labour and did it work for you?

Hi hun. That is an amazing cervical length u have there. Has this consultant been the same one throughout all your pregnancies? I personally would seek a second opinion. But I must say that deapite your amazing length, the history of prematurity may cause him to want the stitch to rule IC out. To my knowledge IC doesnt normally show until mid second trimester and beyond. So though you have a great length now, you may or may not have it still. Was there any given reasons for your previous history at all such as infection/pre-E?
 
You know I was wondering the same thing with my first pregnancy I went into preterm labor at 23 weeks and then was on bed rest because my cervix continued to shorten and now with this pregnancy I wonder if my doctor will suggest to just get a cerclage because of my prior pregnancy???
 
Hey Hun, a stitch definitely will not prevent preterm labour, it only prevents a weak cervix opening up under the strain of an expanding uterus - a totally different issue to preterm labour. Incompetentt cervixes open without the influence of regular contractions. The stitch does not stop labour and cannot prevent contractions.

Was a reason ever given for the prem delivery of your twins? Twin pregnancies are complicated, and ladies deliver early for a whole host of reasons - the cervix simply giving way under the increased pressure of two being one of them. Sometimes a cervix can be totally normal but behaves like it is incompetent just because there are two babies and the extra strain is massive. The same woman can carry one baby without incident and is fine in future singleton pregnancies.

It won't do any harm to have a stitch, but in your case I can't see a definitive reason for having one. Even with the twin pregnancy, you delivered past the usual 'IC danger zone', thus suggesting a weak cervix isn't your issue. That said, it could be that it began to shorten weeks before delivery (as early as 20wks), because it was incompetent. Whether this was a genuine case of IC or purely 'twin related' is difficult to call - which is why your doc is edging his bets and playing it safe.

Your cervix looks brilliant at the moment, and you carried your first baby almost to term so I doubt IC is your problem. However, it can't hurt to get the stitch if there is any doubt at all about your previous preterm delivery. It's better to have a proactive doc who is being overly cautious, than a doc who won't acknowledge IC as a diagnosis thereby putting your pregnancy at risk no matter how small.

I would however clarify if you have a greater risk or not of preterm labour for other reasons known to your doc, in which case a stitch won't help and I would definitely query it's use. It can't stop preterm labour, only a weak cervix from opening. Good luck xx

Hi Cass, he should certainly be considering it. When you say you went into preterm labour at 23wks, I assume you mean your cervix was showing signs of shortening and/or dilating? If it did so without strong, regular contractions then there is a reasonable chance it is incompetent and could benefit from a preventative stitch placed around 12wks. It should certainly be scanned regularly from 15wks to check on its starting length and if it is shortening significantly over several weeks. If it is, then an emergency stitch can be placed well before any catastrophic changes xx
 
Yes my cervix was shortening and I was having contractions not painful enough to feel but they were definitely there and i was thinning and shortening. I will be going in August 28 which I will be 12 weeks for an ultrasound and appt with my doctor I'm hoping he will check my cervix to make sure that it is still holding strong or will at least recommend a stitch to prevent the cervix from changing. Ho ladies thanks so much for replying.. with my first bsby (3rd preg) i started to hav backache and irregular tightenings at 35+3 then full blown labour and delivery at 36+6 in which i was 4cms wen i got to the hospital so that suggests to me that i initially started dilating at the 35weeks but it was not without pain..

With the twins i had braxton hicks from around 18weeks but wad just tild to rest more and give up work.. then at 25+6 my waters broke so was in hospital untill delivery at 27+3.

I hav no explanation to my preterm births..they have not got one for me and are therefore recorded as "spontanous"

I hav had the same consultant throughout all pregnancies and akthough twins is so much more highbrisk and more pressure he still thinks there is something in my nature for preterm labour. I had a chat with my midwife about it for her opinion and she thought it was a good idea also. I see the consultant again this morning to talk it over and get a date and time for it to be done. Although deep down i dont think it will benefit my cervix is its good right now... i think its more or a precaution if anything else.

CERCLAGES ARE SUCCESSFUL! DON'T BE AFRAID!!! I've been reading some blogs lately and I'm horrified to see that MANY women have second term losses and have been diagnosed with Incompetent cervix or are confused because they had an infection of the membranes and doctors don't know if the incompetent cervix allowed the bacteria up there or if the infection happened and then the women just went into pre term labor. I am AN EXPERT ON THIS SITUATION. DEMAND AN EARLY CERCLAGE at 12 weeks of any subsequent pregnancy, whether your diagnosis was certain or not. DO IT. DO IT. It's scary, but you will not lose your baby during the procedure, the doctors who do these procedures are EXCELLENT. Waiting to see if your cervix will hold later in the pregnancy and then bed rest or an emergent cerclage ARE NOT SUCCESSFUL in A LOT OF CASES. A 12 WEEK CERCLAGE will give you the most opportunity to hold your babies if you in the above categories. I also had intramuscular injections of Progesterone, the oil, injected weekly by my husband. The shots were successful for me and I haven't tried any other way because my doctor said this was the best and only way to get the progesterone where it needed to be. I also refrained from SEX and BATHS during the entire pregnancy. DO NOT TAKE BATHS!!! Some doctors won't tell you this stuff! They say you can have sex a few weeks after the cerclage is placed, don't chance it, not even with condoms because if you have group b strep or other nasty things in your lower vagina, anything in there can push it up to the cervix. I don't even like vaginal wand exams after the cerclage and my office will not do them. Advocate for yourself. FIND SOMEONE who does CERCLAGES and is SUPER CAUTIOUS if your current OB-Gyn is the wait and see type and doesn't want to do cerclages. I'm APPALLED by a story I just read on a blog, the women lost a set of twins to the circumstances above, which was my own as well (singleton though), and when she got pregnant with a second set of twins, a doctor should have IMMEDIATELY set her up for a cerclage. Multiples are saved as well from this procedure and I can't believe there is any evidence out there that this isn't successful with multiples as well. Women with multiples may have more restrictions with a cerclage to keep extra weight off of the cerclage but I've read about women carrying twins having a cerclage and she worked until the very end and had no issue. Cerclages SAVE BABIES. Plus, it was a relief for me so have it in. It held tight and I promise to put my experience on here soon. DO IT! Don't be afraid, Cerclages are amazing and the risks don't outweigh the benefit=living baby. I have LOADS of information so in the meantime if you need to email me so we can chat about it personally, please please, email me. I will help you make this difficult decision. I had the worst case scenario, group B strep and other bacteria in the vagina and and incompetent cervix. Scary. But I have two living children and wonderful pregnancies and full term deliveries using a cerclage. I have no damage to my cervix and with my second son (first we lost at 21 weeks to IC and infection of placenta and fluid) and I had the WORST CASE scenario when at 11 week, I went in to have my cerclage placed (I opted to stay awake, control thing) and I heard the doctor say in the middle of putting it in, this is the shortest cervix I've ever seen, I'm going to have to pull it to give me something to work with and put two in, and he had to do the Shirodkar which is an older and more invasive procedure. So I am the worst case. I think my short and effaced cervix at that point was due to the fact that we got pregnant three months after losing my first and that wasn't the smartest thing to do. My uterus and cervix were not ready. But I had a cerclage placed and it did it's job. Email me for more info. and my next post will be about my cerclages. For my third son I had a McDonald cerclage at 12 weeks, so I've had them all except a transabdominal. Luckily we didn't have to go that route but we absolutely would have if it meant keeping my baby in until term. And I went to two different surgeons because we had moved from Florida to Virginia. The care was terrific at both places, cerclage placement is more common than you think. Advocate for yourself, it is YOUR DECISION whether or not you have a CERCLAGE PLACED. IT'S YOUR DECISION. YOURS, NOT THE DOCTOR. If they say it's their decision, pack up and go to another doctor. If you've had a 2nd trimester loss or an almost loss (you had a emergent cerclage or bedrest kept your babies in ) and there was any question as to whether or not your cervix was involved-you have the RIGHT to REQUEST A CERCLAGE at 12 weeks in your subsequent pregnancies. Even with multiples, YES!
