WEll.... it way ALL MY FAULT he was falling out.
I failed to see that the straps need to be thread through the plastic openings and then onto the metal bars, I just put them through the metal bars no wonder they were falling off.
yes... quite embaressing...

*scuttles off in shame to hide in the corner ....
