Mixing breastmilk with formula = LO in pain?

Last night as I had only managed to express 2oz (LO usually takes 3oz) we decided to try adding 1oz of Hipp formula to it to gradually introduce formula. Straight after the bottle LO was in a lot of pain. Screaming, crying and arching her back. She doesn't do this after bottles of just breastmilk. Could it be the formula doesn't suit her or are you not meant to mix them both in the same bottle?

Assuming you made it up with the right amount of water, my only thought is slightly different - our LO had reflux and that was EXACTLY what she was like.

Our dd is ebf except for once and she screamed for hours with bad gas. I felt sssssssooooooo bad and am now scared to ever try again so things better keep going well with the boob.
