PLEASE HELP!! need advice over my childs passport

Hi all :)
I have a 4 year old son (born 2007). I am wanting to take him abroad in November for the first time however my ex is on his birth certificate. He left us when he was around 5months old as we have not seen him since. His parents have moved as well so i cannot trace him at all... He's changed his employer also. Can i send his passport form off with his details and a cover letter to explain my situation or would i need him to sign?? my holiday is booked and paid for and not due to fly until October 2012. PLEASE HELP. thank you for your time xx
 
Are u in the UK? As I am, & I got my daughters passport with no probs and his also on the B/c. I just gave my information, & just put his name & DOB(do u have that info?) & then I gave my parents details in the extra info box & then I wrote a covering letter aswell... & got it with no queries or anything.. I'm not sure if the covering letter is necessary but I would do 1 incase.
 
You can leave fathers details blank and write the reason being that he is an absent father and cannot be traced.
