Why do I always fail at baking bread!?

I always follow the instructions EXACTLY as they're written. I know my yeast is still good, because I just bought it a month ago, and it definitely "bloomed" in the warm water. I'm extra careful in making sure that the water isn't too hot for the yeast. Specifically, when the dough rises the 2nd time, it doesn't rise enough, and the bread ends up being flat and not fluffy enough. I've had this trouble when I make rolls and loaves. Someone tell me what I'm doing wrong! :growlmad:
