Advice on what to ask at final fertility appt?

Hi ladies,

We have our final apt at the fertility clinic in 2 weeks time before we are put on the waiting list for treatment. My background is that OH was diagnosed with a low sperm count of 19 mil and motility was around 35%, feedback we got was when washed there were only 2 million swimmers and so we had to go straight to IVF.

My OH has since had another SA which had a count of just over 19 mil, motility was 50% and washed sperm was 4 million. We were told that we now met the criteria for IUI. I then had a lap & dye and had some mild eno removed.

This is now the follow up appointment where I assume they will talk to us about going on the list for IUI.

Is there anything specific I should be asking here or are there any other options/treatments etc I should be requesting?

Any advice would be hugely appreciated as I am feeling a bit down about it all and really dont know where to start!

How old are you - do you have time on your side?
I would ask what your chances are with IUI and if it is worth going straight onto IVF.

I turn 34 in September! Should I be pushing for IVF?

Hi Macca - good luck with your appointment.

Suggest:
- Bringing a copy of any test results you have for you and DH
- Checking to see chances of IUI
- The FS view of whether you should go straight for IVF
- Other treatment options
- Treatment costs
- Anything you should do whilst trying IUI (any supplements etc)

I assume you have had an HSG completed?

I think with that SA, my doc would say straight to IVF. My OH had a normal - but not stellar - SA, but when he "contributed" for my first IUI he only had 3mil mobile after washing. Doc said it could be a fluke, but if next time we don't get at least 10mil she would not proceed with IUI because less than that is slim chances. That 3mil didn't do it for me. I'm on 2nd round IUI ...luckily we got 10mil. this time.

MK8 I had a lap and dye last month and had some very mild endo which was removed. Otherwise everything looked ok. We are receiving treatment via the NHS and im slightly concerned that they are pushing us down the IUI route because its cheaper.

Punchy, the fertility nurse we met with said they needed 4-5 mil washed sperm to be in with a good chance. As i said before i think the NHS are keen to push us down the cheapest route rather than put us straight on the IVF list.

Thanks Punchy, I think I will really quiz them on my husbands results and get all the facts on his numbers and if they seem low then I will push to go straight on the IVF list.

Hi macca just some more food for thoughts. My FS told us that they would do IUI with minimum 5 million but even then he put pregnancy success rate at 14% each IUI cycle . I thought this was so low.
Another thing you could ask is whether they will do the SCSA test for your DH . This tests for sperm quality for Iui/ivf and DNA fragmentation . Sperm with DNA damage makes it harder to conceive and may cause miscarriage.

Thanks aleja, I will ask for the test and if they wont give us it then I may pay for it to be done privately as this info would really impact on the decision we make.
