Asking to reduce hours

How did you ask your boss for reduced hours? I'm a nurse and previously worked full time but would like to drop to 16 hours/week. I have a form to fill in for flexible working but I'd like to discuss it with my boss first, since you can only make one request a year I'd like to be fairly confident it will be accepted before making it IYSWIM.

So how did you go about asking your boss for reduced hours?
 
I emailed mine yesterday detailing why I was requesting it and she has sent me the forms. Here we have to go through the procedure and submit an application before we would know as it has to go up through management. I am working on my 'reason' at the weekend as she says to get it as clear and concise as possible so as to convince the managers.
 
I spoke with my managers one on one. They were completely ok with it. It may help that they are women and we have a closer relationship than most people do. I just assured I would still work hard. It also helps to have a doctors recommendation to cut work hours.
