Baby suddenly fussy when feeding and stops after five mins

Breastfeeding was really easy from the beginning for me but all of a sudden, in the past two days, it's gone wrong. This has coincided with more fussiness in general. My baby latches on fine and feeds vigorously at first but after 2 to 5 minutes starts crying and beating me with her fists. I burp her and try to put her back on but even if she latches on she starts screaming again in seconds. She seems to want to be held upright so perhaps reflux. We have tried uphill feeding until after letdown but he still gets upset, particularly immediately after letdown. I am terrified she's not getting enough to eat and the frequency of her feeding has not really changed so she seems to be eating much less the same number of times! Also, during night feeds because of the screaming we have to hold her and rock her to get her to go back to sleep - I'm sure creating bad habits. Before the past few days she would either fall asleep while feeding or be so calm she would go quietly to sleep on her own at night.

We tried a pacifier in the last week and finally got her to take it though I've stopped giving it now for fear of nipple confusion. I also started expressing for a few minutes after one feed a day in the past week, though she has not had bottles. Finally, she is six weeks on Tuesday. Is it a growth spurt? Nipple confusion? Fast let down? Over supply? Reflux? Under supply? She has enough dirty and wet diapers. All the breastfeeding websites give a billion reasons this could be happening and I don't find them helpful. We see the doc on Tuesday for her six week appointment. Please help! My husband has suggested giving a bottle of the expressed milk to make sure she's getting enough but I am worried that if it's nipple confusion from the paci that this would kill breastfeeding. Also, I'm pretty sure it's not undersupply as if I squish my boob it sprays milk in two directions, whoa!
 
Sorry about your problem, I can certainly relate with my 8 week old.

He has been diagnosed with GERD and is on medication to help with the reflux but it was very obvious that was the problem. He would spit up after every feeding and scream in paid for hours AFTER I would feed him. Feeding actually soothed him because the milk felt good going down so he would always want to feed. Meds have solved all of those problems!

But now he is latching on and feeding fine for a few minutes and then crying and getting very fussy at the breast. I'm wondering if he may just be impatient and wanting a faster ow of milk because he will stop crying if I squeeze my breast to help increase the flow. Maybe try that with your little one!
