Blast or not to Blast that is the question!! 2/3 day transfer vs 5 day blast transfer

Hi girls, I am currently trying to decide which is the best way to go in regards to which day to do the embie transfer. From your experiences or knowledge, does anyone have any advice on which way would produce a better result. They collected 6 eggs yesterday (Thursday) from me and all 6 have fertilised.

Any feedback in regards to this topic would be greatly appreciated!!!
 
I believe 5 days is when they reach the blastocystic? stage, which obviously give them time to rule out the weaker, or lesser quality eggs. That way you know that you're getting the strongest Embryo = best chance.

At Sydney IVF they automatically wait 5 days, until the blastie stage.

So they are giving you an option?

Blastocysts have a higher success rate as it allows for the healthier ones to be selected. Also they are inserted straight into the uterus so I would think the possibilty of an ecoptic pregnancy is much less.

yeah, initially ET was going to be tomorrow (day2) but I rang the andrology lab to ask them about day 5 blasts etc. The scientist rang me back and booked me in for Tuesday for 5day ET and they will call tomorrow incase tomorrow looks better. So we are a little uncertain on which way to go. How many do they transfer at Sfc?

where do they insert day 3 embies? my tubes are stuffed so no good going that way??

I found this information on day 5 blasto

My own clinic told me that they do do blasto transfers, but every case is not suitable because they say if an embryo is doing so well and is very healthy at day 2 or 3 there is no real benefit in leaving them to go to blast stage, if the embryo's are doing well at day 2 or 3 the best place for them is back inside you! They also said the risk of leaving them to go to blasto stage is that once back inside you they won't grow anymore because they have been left outside your body too long.
 
When my eggs were retrieved, the consultant said we qualified for day 5 transfer, and the literature they gave said the clinic recorded an extra 10% succes rate with a blasto - as the other ladies say, its down to natural selection, as the best eggs make it to blasto. Also, blastos are more hardy for the freezing and thawing process.

The literature also said the womb is more receptive to an ivf day 5 ET than a day 3 as in 'nature' the blasto enters the womb around day 5 anyway and (this is the bit i get wrapped round my arse....) something to do with the right acidity levels / better host????

i really do want to add though that day 2 and day 3 transfers can also be very effective in a succesful H&H 9 months - i'm just sharing with you what my clinic shared with me - it has to be down to each individual, eggs, clinics, lots and lots of other variables - so please dont be disheartened if anyone has a day 2 or 3 transfer - loads of ladies here will vouch for that, i'm sure.

As I understood it, day 5 is around the time the fertilised egg would enter the womb so the conditions for implantation (what the embryo does as a blastocyst) are as close to with a naturally fertilised egg.

I was also told that blasties have about a 10% higher success rate but, as Mrs F says, many, many pregnancies result from 2-3 day transfers and this was how it was always done until they improved the culture medium they use to 'grow' the embies.

It is also my understanding that the reason a clinic might opt for a 5 day transfer would relate to the likelihood that some would reach blastocyst stage.

At the end of the day, it's up to you. There is a certain amount of risk with leaving the embies another couple of days (as you may lose some) but if it all looks good for them, I would go for it personally.
 
It depends on the incubators and culture mediums as to whether your clinic has the capabilities of growing to blastocyst stage. I've had day 3 transfers so far, and will be having the same this time too. I don't think it really makes a lot of difference unless you've got plenty to freeze, which so far I haven't. There are some FS that believe the earlier the embies are put back into mum the better. I've learned the hard way that if it's meant to be it's meant to be, and regardless of what we do or don't do, it's not going to make a huge difference - it really just depends on how you think you'll feel if it doesn't work.

Thank you everyone for your thoughts on the matter. The scientist called this morning and said there are 4 embies that are 4 cell, and the other 2 are 2 cell. The scientist is really pleased with all 6 as they are currently looking healthy. He says he would suggest to take them to 5 days, as he cannot pick out which ones to transfer at the moment as they are all looking good. He said if there were a couple outstanding ones over the others he would have suggested today. So...FX and they keep growing .

So I guess they have assessed the embies and so far they are healthy and going well which is good. Otherwise today would have been ET day.
