Hi :wave:

I gave birth to our son at 23.4 weeks two years ago, for "unknown reasons" there was an infection (chorio) but it's unknown if it was the cause of my PPROM or as result of the PPROM. Either way my doctor is sure I will need a cerclage, which I do want. My question is though...can I demand the one I want? Or will they make the choice for me? I ask this because I know the McDonald can still allow for your cervix to shorten and funnel.

I would prefer the Shirodkar but I know this is less common, however it holds the least chance of infection. And from my research chorio may have a genetic factor as a lot of women get it twice. However, I think I may get a big fat "NO!" answer as it some times requires a c-section. Buuut that being said, I am pretty sure if this baby does (fingers, toes, everything crossed this baby stays put for 40 long miserable weeks :happydance:) go full term, I will need a section anyways. Because I had an impossible time getting Lakai out, TMI but there was tearing and I was fully dilated and everything.

Can I demand the type of cerclage I want? Has anyone else successfully done so?

Hi there hun.

I had a shirodkar stitch in my last twin pregnancy, and I wouldn't have any other kind if I were to have another baby. You are right, it is a better stitch, primarily because it is placed higher on the cervix and is sewn thru the vaginal and bladder walls. It got me to term (39wks) with huge twins who were 9 and 7Ibs, and tho I funneled to it, it didn't give way in the slightest.

I was fortunate that my consultant insisted on that particular stitch becuase I was carrying twins, and she is an expert in placing them. I am certain tho that you can atleast request one, and find a consultant good at placing them (not all consultants can or will do them).

As for a section, you no longer have to have one because of the shirodkar. i had a section, but only because my leading twin was breech, otherwise it would have been removed at 34wks and I would have waited for labour to start. Because i had to have a section the doc said she would remove it during that procedure.

PS - As an aside if you do have to have a macdonald, please be assured that it too is a perfectly adequate stitch which when placed electively is brilliant at preventing IC. The main thing is get a stitch put in before any changes to the cervix. Didn't want you worrying excessivley if you have to have a macdonald ;)
