First fertility appt

Hi ladies, I have my first fertility consultation tomorrow... I am nervous but excited! I am wondering if i need to bring anything or have any information written down. I want to make sure things so smoothly and I am prepared. I started ttc in sept. Got pregnant in October. Lost that baby with a mmc in December... Had prolongd bleeding and after 9 weeks of bleeding finally had a dc in lat feb. Have been trying since and no luck. I have positive opks monthly... Every month I am sure I am pregnant and always am not. It's been 6 months since dc and I am scared that maybe something happened at that procedure. Any advice would be great. Thanks ladies, I hope we all get bfps soon.
