For anyone getting their grant forms signed by a GP

I don't have a midwife so my GP whose in charge of over-seeing my pregnancy signed my SureStart and Health in Pregnancy Grant forms. I got both of them returned to me today with yellow post-its on them saying 'Please Confirm Midwife's Number'.

Little upset because I called up before I sent them off and asked if -anything- needed to be sent with the forms and was told that so long as my 'Health Professional' (It also says Health Professional on the forms, not 'Midwife.) had signed it, I didn't need to send anything with them. It also doesn't state on either of the forms that if you don't have a midwife you have to send proof that you don't have a midwife.

Just a warning to others then, I've had to go down to my Doctor's today and get a letter confirming that I don't have a midwife and so hence cannot add a midwife number. Half tempted to complain to someone because my GP signed it, filled it out and stamped it with my surgeries stamp - a quick call could have confirmed that they were a doctor there and I was on GP care :( Or if the forms had said to be signed -just- by a midwife I would have gone and got it signed by the midwife I was originally with.

I really wish someone had told me when I called or it said this on the forms as I've been waiting two weeks and now have another 3 before I can go baby shopping ;_:
 
I think it should NOT have been sent back! The form needs either a GP signature and practice stamp OR midwife signature and pin number.
