my sil has a 4 week old that is combi-fed and has pooped like 20 times already today - as she has all week.
All this pooping has led to a bad diaper rash cuz she can't keep her dry. Shes considering switching to JUST formula for a few days to help, but Im afraid she'll get engorged..
any ideas?

I wonder if she is feeding the baby to much foremilk and not enough hind milk. Ask her how long she bf for on each side. I think its something like to much foremilk can make a baby's tummy to acidy. Have a google and read about it. I realised i was switching between breasts to soon as soon as i decided that was the problem i stayed on same breast if baby was hungry again within 3 hours. She quickly didn't mind i did feel a bit too empty but my boobs soon adjusted.

Could she call a health nurse? they should be able to give her suggestions to help with it. I would be worried about baby being dehydrated.

I agree about the fore/hind milk issue. Also if she is using wipes, stop for a bit and just use water and cotton. When my LO was 4 weeks old he had a diaper so bad his skin was peeling off :( I tried 3 different rash creams before I found Penaten, and that cleared up the rash in 2 days.

I agree with foremilk/hind milk imbalance.

I found an article about "too much soup and not enough cheesecake". Basically the foremilk is like "soup"- thin and not filling and runs right through the baby, and the hindmilk is like cheesecake- thick and filling.

She should try feeding for several feedings in a row on one side before switching to the other. Here is the article: https://www.normalfed.com/Continuing/gulping.html

I agree with PP about the milk.

For Diaper Rash: baths in water with baking soda and try coconut oil instead of cream - that stuff is amazing. xxx

I have no idea about the milk but for nappy rash we use Metanium. It's amazing stuff.

Apparently shes exclusively pumping and she can see it separate, so there is lots of hindmilk

Soak the baby's butt in warm water. It will hurt at first but it always helps.
