Help me read these hormone values please

Hey guys.
My best friend and her husband have been trying to have a baby for a while. She was finally able to get some blood work done after a few years of run around with her Dr. and this is what came back. However getting back to see the doctor is going to take a few more months. (I know she needs a new doctor)
She has PCOS and Wondering if anyone has any insight to these for hormone values.
This was in day 5 of her cycle

The values on the left and middle are the reference values, her value is on the right and should be anywhere between the two reference values.

Everything seems to be in the normal range apart from the BMI. I'm not an expert though and can't say anything for those values that don't have a reference value indicated.
