How do i ask work about maternity leave?

I think I am eligible to apply for maternity allowance.
I am currently in my probation period and I intend to leave my job before my probation period is over. I highly doubt that I am going to get maternity leave but I need my employer to complete an SMP1 form.
Im about 21 weeks and I want to leave in about 3 weeks time when I will be 24 weeks, I have accrued two weeks holiday. 29th july is when i am 15 weeks before my due date

Am i supposed to write a letter asking them to give me maternity leave first and then they give me the SMP1form back refusing it which I then submit with my application for maternity allowance?Or do i just give them my Matb1 form and then hand in my notice ?

Any advice or letter template greatly appreciated.

if you are leaving your job for good and applying for MA, you don't need to write to ask for maternity leave.

You will just have to write your letter of resignation, with no need to mention mat leave
matb1 goes with your MA application

You won't be taking maternity leave, you have to be 29 weeks before you can start maternity leave so even with your 2 weeks holiday you would be leaving your job at 26 weeks

Hi, thanks for the info and sorry if i am asking dumb questions.. i'm totally clueless about all this! Do you know if i still need to get my employer to complete an smp1 form before i leave my job ?As far as i understand a completed smp 1 form is required when applying for maternity allowance...
 
hello, don't worry if the questions seem daft.... it can be very confusing!!

you need the SMP1 form if you are employed by someone when MA start
ie. HMRC want to know why they (your employers) are not responsible for your mat pay.

As you are classed as unemployed it shouldn't matter.

I applied for MA but I was self employed so it was a bit different

I'd get the MA forms and have a chat with the CAB but as far as I'm aware, you just need to prove your earnings for the test period ie relevent pay slips
