ID sent with SureStart form?

Quick Question: Just finished filling out my Sure Start Form for the grant so that tomorrow I can get it signed by my doctor and get it sent off.

The checklist at the back states 'Check that you are sneding all the documents we have asked for'

I've read the form a few times and checked that it hasn't asked for any documents - (in fact it doesn't seem to mention, anywhere on any question even the sections I've skipped because it doesn't relate to me) but thought I'd ask you girls if you just sent the form in as well?

Didn't want to send it in to be rejected because I hadn't sent ID of some kind in!

If youre getting the certificate bit signed then i dont think you need anything other than that.
 
I didn't send anything with mine and I got mine. They can check what benefits you get on their end, so you don't need to send proof of anything like that.
 
i asked the job centre about this and they told me some people send their maternity certificate (if they have one because i dont) or childs birth certificate (if child has been born) but if GP or M/W signs back there is no need to send anything and if they need anything they will phone or write to you
