Hi,
I really need some advise as i am do down and upset and think my doctor is just fobbing me off, ok so me and hubby have been trying for baby number 2 for over 2 years now ( it took us over 5 years to have first one, only got pregnant as took Agnus Catus first time but dont want to take it again as makes me feel sick ) so Ive had scans and it shows I do have mild PCO and i dont have periods at all, though all my bloods have come back fine and i have a healthy BMI... Ive been to doc and asked to me referred to a spealist but he keeps saying he needs to stick to the guild lines so made hubby do a sperm test and the results where as follows:
Motility 44%
vitality 85%
count 118 Million
Morphology 4%

Now he said that everything was fine apart from the last one and he said that hubby will need to retest in a month before he can refer me as the fertility specialist wouldnt see us with a count this low, though i have been reading on the net that nowdays even though this is low this is the average for men? but my doc wont do anything and wont refer me untill my husband has done another test... is he right or shall i get a second option? i just cant get over how hard it is to see a gyno. surly he should refer me??

The morphology is the one that is borderline, not the total count - right?
There are 2 scales for measuring morphology, one is 'basic' ie does it have a head and a tail and the guidline for normal for that is 15%. The other scale is very detailed, on the size of the sperms head, length and shape of tail etc and normal against that measure is 4%.
Either way I don't see your DH's SA as a major concern, it is common to retest and get a 2nd SA though because they do change and perhaps this is what your Dr means - your area require him to do all the basic tests first and if a count is borderline he must do 2 maybe?
Unfortunately every area varies (in my area the GP does virtually no tests they are all done at the specialist).

I'm guessing he wan't the 2nd test to decide whether to refer you as the patient to 'gynae' (with your clear tests) or your DH as the patient to 'Uro' maybe?

I would probably go with the 2nd test, it'll only be a short delay and take it from there. Do bear in mind that in most areas once referred there is very little they will do to help someone who already has a living child unfortunately. I believe the most support you would get if all your tests are fine might be clomid.
 
Sorry I missed that you don't have periods, so I guess he will refer you to gynae anyway, but just needs to have more information to send with your file maybe?
 
Hi i know its frustrating but before the FS Would even put us on their waiting list, we needed to hand in two SA a month apart as well as having numerous blood tests. Try not to worry. In a way the guidelines save the patients time as they would just send you away if the proper tests hadn't been carried out. X
 
hi thank you all for the advise, I think im just getting impatient, will try not to think much about the next test and hopfully that way it will go quicker, will keep you all up to date, also all i want is clomid would they give me this on my first appointment once i see the gyno? xx

the other ladies are right each area is diff where i am i had 2 have bloods + a u/s b4 i could b referred then wen referred i have had 2 have more bloods dun + a hsg ordered by my fs wen i go bak i hope 2 find out wots next. as for SA this has yet to b tested for us as we r using a donor so am guessing its being left til last.

as for clomid i am not sure if a gynae would prescribe it straight away they may wish to perform more tests esp is ur mild pco diagnosis was some time ago

Hi, I had to have all the blood work done twice and my hubby had to have an SA done twice also. Then when we were finally referred and got the appointment we had to have them all done again and were given Clomid on our second appointment.

Each area and doctor is different though.
 
I'm in Edinburgh and I couldn't get referred until my oh did 2 SAs, one month apart (or rather the ERI wouldn't accept the referral and put me on the wait list till then). But that was because my GP referred me as a fertility case rather than just to investigate my lack of periods.

I don't think it's good to go a long time without periods, so I'd push to get it looked into. But given that you want to get preg, i think you'd be better getting the other SA done and get referred to the fertility centre.

The waiting is the hardest part, it does sound normal though, it took us months to get through all the tests its so frustrating.

If you are not having periods you won't be ovulating and this is probably the issue for you and DH, 4% morphology is ok as far as we were told there are mixed opinions on this though.

I am sure if everything else comes back fine you will be given Clomid, I think the issue Drs have with giving clomid is its known to be over presribed and they like to make sure there are no other barriers to conception before they will give it these days... this is what I was told, but again depens on your Doc.

Clomid has side effects though, you say you don't want to take Agnus Castus again because it makes you feel sick, be aware that clomid has a whole list of side effects which worsen with a higher dose.. if it helps you get your baby though its worth it.
