Yolk sac but no fetal pole

Hello everyone I had my LMP 11/28/11 by my count I am 8 wks preg. However I went to the dr today for a my 8 wk check up and she did a vaginal U/S but there was a gest. sac but no fetal pole she said either my count was off or the baby never formed. and that I should look out for bleeding. Has this happened to anyone else?

This also happened to me about the same time about 8 weeks and I miscarried back in October.. TTC again now..
