My midwife gave me the helpline number for it as she didnt have the form.

They told me they only distribute to Midwifes so i left her a message saying she will have to get it.

Shes just rang me to say she has recieved then and has filled out one for me thats been left in the docs reception!

Result!!!!!

Pointless thread really but just wanted you girls to know its not impossible to obtain. Give your midwifes a big kick up the bum!!!!

Thats good to know. I signed up for the free reminder on the website and guess what I am now 25 weeks and no email!!

Thankfully I know and will definitely remember when I see my MW at 28 wks.
