Anyone Want To Help With My Birth Plan?

Husband is sleeping (he just got off of 24 hour shift at 930 this morning) and I am bored so I think since I have not done a formal birth plan I may need to get stepping, as my next appointment is at 36 weeks. I believe doctor looks birth plan over at that appointment, as well as I am suppose to have it on my person (in purse etc.) every day after that according to doctor, so that I always have it in case baby decides to be born early and I need it.

I have started several, but they never seem to be what I want, or how I want to say it, and I am really not sure what all needs to go in it.

Any suggestions? Thanks!
 
My hospital just had me fill out a sheet that I checked if I wanted things or not....and fill in the blank for everything else..like who would be with me in labor. What kind of pain relief if any, do I want them to ask about pain relief or not bring it up. Do I want to use the tub, etc. Do I want to eat during labor? Do I want to have a mirror to watch the birth, or touch the head while crowning. Holding off on things for 1 hr after birth for bonding time. Who cuts the cord? Who gives the first bath? Any special concerns or requests and how do we want our labor and delivery to go?

Yep :) Same one I just found :)

I never really realized how small "one page typed" is. I mean seriously they only want you to have 1 page? Maybe if you do not put proper spacing it "Might" work, but I just filled out this birth plan and it is 6 pages if printed to a PDF. : / Hope my doctor is flexible. :)

at my 36 week appt the lady wrote down the stuff. like pain meds, whose going to be in room..
