avoiding blood in milk

So I was struggling with bf my first but the thing that stopped me in the first week was lo vomiting up a whole feed of blood and a two day stay in hospital. My nipples were sore but fine and it wasn't till I used the pump in the hospital and a few ounces of blood came out followed finally by milk we realised where it was from.

I want to try again with my second but the memories of that are quite upsetting, I don't know what I did wrong to cause the damage but can I avoid it this time round?
