HCG levels and what you see on ultrasound

I have a question b/c my doctor isn't giving me the information to answer this question.

I went to the emergency room this morning in severe pain, mainly in my right ovary but I would get it in my left & near the middle of my uterus. I'm also spotting brown.

I was diagnosed with multiple cysts on my right ovary, and with a threatened miscarriage. My ob/gyn told me yesterday that it seems like I'm miscarrying. But obviously that's not true, yet.

My blood work has doubled perfectly in the last week, going from 2,117 on Friday to over 10,900 this morning. I had an emergency ultrasound done this morning, and all they saw was the gestational sac & a small yolk sac. No baby, no heartbeat or anything.

Doctors at the ER say that everything looks perfect except for the spotting, hence the diagnosis of threatened miscarriage. My question is, though, since my HCG is over 10,000 shouldn't we be seeing baby & heartbeat on the ultrasound, already???

I'M SO CONFUSED!!!!!!!!!!!!!!!!!
 
Greats, don't know too much about this but wanted to tell you what I do know. I had an ultrasound done at 5w1d when my HCG measured 9912. At that point all we saw was a gestational sac and yolk sac. No fetal pole, no baby and was definitely too early for HB.

How far along as you? I hope the spotting settles and everything works out for you!
 
It really depends. There is a rule of thumb that over 10,000 you should see a fetal pole. I went in for some spotting at 5+4 (only 5+1 by my conception date) and judging by my hcg levels and doubling time my NP was convinced we would see a fetal pole with a heartbeat. I knew it was too early and guess what it was! 2 weeks later we saw a baby with a healthy heartbeat of 145. I think some people just have higher levels of hcg and really dates and sizes of the sac are better indicators than the hcg.
 
Based on my LMP I'm 7w1d, but I used to usually have a 35 day cycle so technically should be 6w1d today. However, my cycles aren't normal, a couple cycles ago I had a 44 day, then a 48 day, then the cycle before this one was only 33 days so I honestly have no idea how far along I am. The ultrasound tech at the hospital dated me around 5w4d so I probably ovulated later than I thought.
 
Well seeing a yolk sac is a good sign, I had a blighted ovum last year and we never saw a yolk sac. The only thing you can do is wait for a repeat scan. FX'd for you!
 
I don't think it has anything to do with HCG and when you will see the baby or heartbeat. I have kept a LOW HCG the first 11 wks and they don't even double near every 24hours...

6 wks, Vag US saw HB
7 wks 29330 HCG
8 wks saw HB via US
10wks 57,590 HCG
11wks heard hb via Doppler
