Help me figure out what is going on please

I need some help. The last few days have been horrible. My 18 month old son has always fought sleep, but it has gotten worse. I know he has a sinus infection and we got antibiotics for it and I'm giving him motrin for the teething (per the ped's recommendation) but he still screams for an hour before he finally falls asleep. Usually he only fusses 2-3 minutes and zonks out, but the last 4-5 days he has been full on, red faced, screaming mad for nearly an hour. I go in there and lay down with him, but it doesn't help. He still just screams at me. I don't think he is getting real restful sleep either. He hasn't woken up happy in several days and always seems so tired all day.

Do you think he just doesn't feel good because of the sinus infection or are terrible twos coming early?

Perhaps the antibiotic is giving him a bit of reflux?

We just started the antibiotic today and it's one he has had previously with no issues. The problems started before the antibiotic.
