How do I book my 6 week check up

Do I just phone my gp and book it with him or is it a nurse? Also do I need to book one for my baby? He's at a different doctor to me as I've just moved to a new area so was told to register him and then register me after my 6 week check up

My surgery sent it to me through the post. You both need to see a gp so give them a ring and book it.
