MATB1 form.........its wrong :-(

Hello all

Had a call from my MW this afternoon about my MATB1. I did have an appointment with her on Wednesday but she cannot make it so she has put my MATB1 in the post.

I am seeing my Line Manager next week about my Maternity Leave. LO is due in 16 weeks.

The problem is that she has put my due date down as the 9th August and not the 2nd August(which is on my scan sheet)........is this going to be a problem or will it be ok. She told me to cross off the date and change it, but I don't really want to do that.

What shall I do? Shall I just leave the 9th on it or change it?

Depends if this is going to affect your leave date or not. If it is, get it changed xx

Would I be able to change it myself or will the MW or doctor have to do it?

Abit worried about this now, as its quite an important document.

I don't think anyone would know the difference who changed it. If your employer questions it, say it was wrong and MW had to change it

Photocopy the thing you got with the scan and attach it with the date highlighted?
