Night time grunting and straining 4 weeks

Please help!

My 4 week old has started grunting and straining and being generally noisy at night. He was 5'weeks early and we had 3 weeks of him being very settled and quiet including at night. He is combination fed with formula and breast milk and now he's a month I'm weaning down the breast and upping the bottles which he takes very nicely. In the past few days he's been mainly formula.

He's pooping fine multiple times per day so isn't constipated. He does get a bit of wind but he burps fine during his bottle and after when we wind him. Just the past couple of nights after his night feed and getting his wind up, he lies in his basket straining and grunting. It can be soooo loud! He's not crying at all. Last night it went on for an hour and I can't sleep through it!

Is it just a phase or is there something I can do?
 
Ok I've changed my mind maybe he is getting constipated...
 
my first baby used to grunt ALL night, drove me insane and he was promptly evicted from our room into his own so I could sleep! it started at 3 weeks and found the only things which eased it was either tummy sleeping or raising his Moses basket slightly.

He never had colic or trapped wind, my 2nd baby is the same, he's just turned 2 weeks old and grunts away all the time, he too has been moved to his own room! ooooh the peace and quite
 
I have a grunter, too. Can't figure out why or what to do about it. At 11 weeks he is not longer doing it every night, but sometimes still does...
 
My lo has this and was hospitalized after a choking episode where they determined via ultrasound it is silent reflux brought on by bloating.
He did have antibiotics after birth and I took them in pregnancy which makes this worse. Him and me now both take probiotics and he gets a simethicone based solution before feeds so he can relief gas (he farts lots when he gets it!)
His grunting started at 3 weeks and then slowly turned into really bad night congestion and choking episodes. He was also wheezing as if he had asthma, plus he started throwing up acid and arching his back and screaming in pain. I was sleeping holding him upright at night so he could breathe.

I eliminated Gluten and Dairy from my diet also, plus the medicines and now we are thankfully back to very minor grunting and getting better.
The reason they grunt is the acid/ throw up affecting their vical cords. Usually when he is sound asleep he does not grunt but when he stirrs he does. He used to stirr a lot from tummy aches and never really settle into a deep sleep.

Maybe you switching him to formula could have something to do with it? Baby's can be allergic. My lo is breastfed but for allergy reasons I cut out dairy as many babys have problems with the milk protein.
