questions to ask the Dr ??

Hi .. I am 35 and we have been ttc for 14 months. I know I have pcos (as I was diagnosed when I was when I was 21 but was never given any treatment)

Anyway I have a Drs appointment tomorrow to get help ttc and was wonder what questions I should ask the Dr and what sort course of treatment I should expect.

i had my 1st appointment last week and my list included the questions what am i entitled to in terms of treatment (how many free ivf tries) what happens next, tests ect...

also questions that were specific to me were what is the bmi limit for ivf? and can a sa change over time (my oh had one previously)
