So nervous - filing for full custody

I met with my attorney last week to start the process of filing for full legal and physical custody of LO. FOB has no idea. I feel like it would be better to give him a heads up that he's going to be getting the petition in the mail, but I feel sick to my stomach just thinking about having that conversation.
Also, does anyone have any experience with this? I'm citing fear for LO's safety if she is alone with him. I want him to have visitation, but want it to be supervised. A few months ago he said he would kill himself if I didn't move back in with him. He also said this to my therapist. So the petition for custody cites "mental health" issues as the reason for my request.
What are the chances I'll be granted full physical/legal custody? Any suggestions or advice/experiences from others with similar situations would be greatly appreciated!!!!
 
I asked for sole custody, but we settled in mediation and he wanted it to be called joint (but visiting is still on my terms), so I said okay. FOB doesn't have mental issues though. Besides his inability to tell the truth or commit to anything in life :) I'm not sure how that will go, but I'd say if you have a professional opinion that he needs supervised visits, shouldn't be too hard. Guessing USA law is different to English law so I don't know what the likelihood will be for you.
