Those who claimed the sure start grant after baby was born

Do you need the Matb1 form to claim after baby is born or just the birth certificate?

Hiya, im sending off my form today and my lil girl is 9 weeks old. It has to be sent within the first 3 months of having your baby. You fill out the form with your details & your baby's and then take it to your health visitor who confirms the information you have put down is correct and signs it x
