How do you know when your boob is empty and baby is just comfort sucking on nothing? She always comes off of her own accord, and sometimes takes the other side after a burp.

Take notice of how often she swallows, at beginning of feed my son has one or two sucks then swallows, I swap when he's having to suck 3 or 4 times

The breast is never empty because it is always making milk. It doesn't have any storage areas for milk, which is why at the beginning it is so uncomfortable and engorged because your body is making milk that can't go anywhere but just get backed up in the ducts. Once supply regulates boobs rarely get engorged because the breasts know how to make milk quickly on demand. In terms of when to swap, I'd say when baby is unsettled and rooting but refusing to latch to that side you should swap.

You can tell when they aren't actively feeding anymore, the sucking will change from long and slow to short little sucks.
