Work have lost mat b1 form

Hi, my work have lost my mat b1 form. I have a photocopy of this and can send them that but will my midwife give me another one?

Also my midwife hasn't marked in my notes that I have received this form. Could I chance my luck and say I never got it or should I just be honest?

Has anyone else had to ask for another form?

Hiya. I'd copy the photocopy you kept and give it to your work. At the end of the day you did exactly what you should have done and they messed up. If it isn't enough then you should contact your MW and explain the situation. It can't be the first time something like this has happened.

i work ina surgery and have had a few woman needed duplicates. I dont think your work will accept a photocopy but best you photocopy it for your records. just ask midwife to do another one shouldnt be a prob xx
