How do you apply for it??

You get a form from your midwife at your first appointment after 25 weeks, she fills in the bits she needs to do. Then you have to fill in your bits and post it off xx

From 24wks onwards.

You will prob get at your midwife appt - it needs to be signed by them.

I have a midwife appt at 26+4 so am getting the application then.

I have heard people getting it sent back to them as they applied before 24wks.

yeah you get a form from your midwife at 25 weeks or over, dont get it before as they will send it back to you xx

Yeah like everyone else, you get a form from your midwife but make sure you ask as mine nearly forgot to give me mine....along with my MATB1 form x x
