Advice on how to get my 4 month old to sleep?

I've previously made a thread on my baby not sleeping and I'm not exaggerating when I say he doesn't sleep!

Yesterday we to OHs family home and he probably slept 5-10 mins max from 12am-8pm when he eventually fell asleep in the car and woke as soon as we got home. Due to the lack of sleep all he does is cry, moan and I'm really struggling at the moment.

This has been going on from just before he turns 3 months and it's getting worse by the day. He use to sleep ok at night but even at night his constantly waking! He doesn't sleep in his bed any longer do we bed share and I swear I'm constantly breastfeeding him to sleep throughout the night. I'm constantly hearing about babies his age who sleep a good amount through the day so to have a baby who doesn't sleep seems insane to me?

Please someone give me any advice? I can't keep getting out of the house just so he can sleep in the car or pushchair. Surely this isn't healthy for him? I'm getting really stressed about this. I'm struggling to even do anything at home like cook, clean and even getting dressed as his fussy and crying.

Thought I'd add, he has eczema so his constantly itching his neck, head and legs plus he has reflux so not sure if this could be causing sleep problems ?
 
At that age, my daughter always slept in a wrap, unless we were out somewhere and she slept in the car on the way or in the pushchair if I wanted to go for a walk. I never really did the stay at home and enforce sleeping in a cot thing, as I hated the idea of being stuck at home or having to tip toe around in silence to not wake her. So I put her in a wrap and she'd fall asleep as I went about my day. It meant I could get everything around the house done and she wouldn't be bothered and she got lots of sleep during the day, which meant she slept well at night (she still woke for a couple feeds during the night, but generally at that age, slept well in between and I think it was because I did whatever it took to get solid daytime sleep in, we also bedshared which you're doing already). In the long run, having her sleep on the go or in the wrap was actually the most convenient thing as it meant we never had to plan life around naptimes. We just did what we wanted and she would sleep wherever we were when we were out and about. It's worth a try, a wrap is also great for reflux and can really comfort them as it keeps them upright. Otherwise, I'd also lie down with her and bedshare during the day too, which was great because it meant I got caught up on sleep too. Then we were both happier during awake times and I could get things done around the house then.
 
I did the same as the PP with both my son & daughter used a wrap/sling/baby carrier they both slept very well during the day and I still managed to get on with things!

My son is now a very good sleeper and will sleep through anything (we never tip toed about etc) my daughter still wakes twice in the night but otherwise is sleeping well
