I used to work at Sainsbury's head office - women would actually phone us up to compain that they had had their first labour pains while shopping in the store, and they had not been given any vouchers!! We would explain it's not an entitlement, but something that the store could choose to do.
Quite often the store manager would send some flowers to the customer or get them back in store with their baby and present them with some vouchers (with a photo opp for the local paper, obviously!). I'd think they would be a little anxious to make sure they were giving the customer all due attention and care, especially in today's litigious society - store manager wouldn't want to give the customer any excuse to claim negligence on the part of the store if anything went wrong with their labour.
I guess the thing about being able to leave with whatever is in your trolley / basket is more logical - it would, after all, seem rather heartless and mercenary to hassle the woman for payment when she has more important things to worry about!
