I asked at the post office for a form only to be told they no longer exist?

Apparently the Post office no longer have them!!

I have always just phoned them up

Have never been given a bounty pack

I was given a bounty pack the day after LO was born.

Ring CB up and request for a form to be sent out to you, usually takes up to 3-5 days x
