How to make a teddy??

I have seen some gorgeous teddies from bubs bears handmade out of baby's first clothes baby gro's etc and I wanted to try and make one for my daughter as I am really arty and think the sentiment would be lovely rather than buying her one..... but..... I have no clue how to start?? I have a sewing machine and can hand sew neatly but not sure how I go about making a pattern or finding a pattern for the actual teddy? I would like it to be about 12"-14" tall. Any suggestions? or help?
