How to put URL in signature?

Sorry, title isn't very good..

How do I go about putting a URL in my signature, but rather it saying the entire URL address, making it a title?? I used to know how to do it, but I can't remember anymore. Hopefully this makes sense! I know a lot of people have like a 'parenting journal' in their signature.. I'm not sure how to get it to have a title rather then a URL. Thanks!

(minus the *)

you would see TYPE YOUR TITLE HERE and it would be the link to the URL you used.
