I apologize if this isn't the right place. I wasn't really sure where else to ask. I'm also sorry this is so long.

In July 2015, I started seeing a new obgyn who finally took my fertility concerns seriously. I went through a bunch of blood tests and ultrasounds over the next six months, although nothing too in-depth. From then until now, I've been diagnosed with PCOS, endometriosis, and adenomyosis. It was also determined that I had borderline-low functioning thyroid and insulin resistance.

As of my most recent (April 2017) blood work, I no longer have an issue with my thyroid and there wasn't any obvious issue with insulin resistance (I lost 50lbs, which helped balance things, I think). However, an ultrasound showed that there were still cysts on my ovaries. And my cycles are nowhere near regular. They averaged out to about 35 day cycles, but some were longer. I do usually end up ovulating on my own, but way past the "normal" cd14.

In addition to all that, I have had multiple early miscarriages now. I had one Aug 2014, another in June 2015, another Sept 2015, one April 2017, and I'm going through another one now. All of these pregnancies ended before 5 weeks. I had one that didn't end before 5 weeks and that resulted in DS2 in 2016. My doctor's office only has records of the ones from September 2015 and the one I'm currently going through. I never got as far as going to the doctor before the pregnancies ended for the others.

Im beginning to think I might have a progesterone issue. And obviously an ovulation issue.

If that's the case, what tests do I ask my doctor for? What can I do to help? Is there anything that will help me get (and stay) pregnant? Would Clomid be something worth asking about?

I just feel so lost and so hopeless right now.

Definitely ask about progesterone supplements. You might want to see an RE to see if there is an egg quality issue.

Have they done a cd 21 test (should be about 7 days after o)? That would show progesterone levels. Definitely ask about progesterone testing/supplements.

I think when I first started going to this doctor, they checked progesterone levels but I don't think it was after ovulation, I think it was right after AF started or ended.

Hi x
Just came across your post .. I had 4 miscarriages around the 5-6 week mark and fertility problems .. I was diagnosed with an under active thyroid and on levothyroxine .. I only became pregnant again when my TSH became under 3 and I was able to maintain that pregnancy by staying under this .. It may not be helpful but worth considering x all the best

I second the post-ovulation progesterone. With long cycles, I think they say 7 days post ovulation, but you can have a little flexibility in there. I had borderline normal progesterone so I was also advized to get progesterone test within a week of a positive pregnancy test. Maybe something to ask about?

Do you mind me jumping in here and asking how long it took for your TSH to get under control? I've been on Levothyroxine for a few weeks, TTC at the moment. Did it change your cycle at all? I am on 10DPO, and started spotting yesterday. Today it is strangest cycle ever, and BFN - but I'm only due to get AF Saturday. Was wondering if the meds have caused havoc/if it's spotting from implantation (wishful), or what? Thanks!!!

I would request to get blood tested on various days of your cycle to determine progesterone levels after ovulation. I too had low progesterone which was not properly diagnosed because my gyno only tested me once at the "average" time for ovulation. Since everyone has such different cycles, you need various blood tests at different days in your cycle to properly determine if your progesterone levels are correct after ovulation.
Given the length of spotting i had prior to my period, I went to a different dr to determine if my progesterone levels were ok and they who ordered the sequence of blood testing and determined my progesterone was in fact low. Now I take progesterone three days after I ovulate until i get my period.
