Maternity allowance form question

I sent off my maternity allowance form yesterday. But I don't remember anywhere on the form it asking me when I wanted it to start, or when I plan on starting my maternity leave. I need it to start as soon as I finish work otherwise I'll have no money.

Does anyone know how they choose the date to start paying?

I think it said on the form it starts from the 29th week of pregnancy (11 weeks before your due date).
If they receive the form after this date then they usually ask you when you want it to start, the latest it can start is the day baby is born.

after mine was acepted and sent back, they phoned me to ask how often and when i'd like it to start from
