nappy rash and bleeding

My little one has terrible nappy rash. I think he's teething. He's dirtying his nappies constantly and it smells like chemicals.

Every time I wipe him his skin is tearing and he's bleeding. Is this something to worry about?

I've taken to bathing him when he messes as much as I can x
