Nurse: "I made a mistake"

Fair enough that you don't want to make a formal complaint, but I really urge you to get an apology for this.
The fact that she didn't bother seems to suggest to me that she doesn't understand the implications of such an error.

Either speak to the practice manager and explain that you won't make it official but expect an apology, or actually speak to the nurse and say 'I realise you made a mistake etc but I feel quite strongly that the admission should have gone alongside an apology' I think that you are that grateful with the negative result that you have let it go, but think about if it happens the other way round to someone....
