Aww I'm so sorry about your puppy. Its such a horrible thing to happen. RIP Brian 
It must be a (relatively) common thing in puppies. My dog in Thailand almost hung himself after jumping out of the jeep after me and getting his collar caught Luckily I saw it and came back. Can easily happen
