Recurrent Miscarriage Thread

Im just going through my 4th miscarriage and I'm hoping someone here has any insight into what is wrong with me or what I can do next - sorry if this is longwinded...

Live birth 2010 - no problems with conception (6 months after pill) or birth.

MC 1 - strong bfps, lots of symptoms, started bleeding at 11.5 weeks and confirmed blighted ovum. Very traumatic MC in hospital, needed blood transfusion etc.

MC 2, 3 and 4 - exactly the same each time - 2ww feel mild symptoms (tiny bit sicky, sore boobs). BFP at 14dpo but a bit faint, obviously there but on the fainter side. Tests don't get any darker, symptoms start to diminish by 5 weeks and between 5.5-6 weeks I miscarry naturally. The only difference this time was light spotting from bfp onwards.

With MCs I got pregnant on cycle 1, 3, 3 and 1. Getting pregnant is very easily - which breaks my heart even more :-(

GP initially did a few tests after MC3 full blood count, serum ferritin, vitb12, red cell folate, liver function test, lipid, thyroid function test, renal profile, blood glucose, LH and FSH. All came back normal although FSH (cd4) was 9 which is higher end of normal.

Thought it was 'bad eggs' so followed a healthy egg diet and supplements for 2.5 months before trying again and MC exactly the same way with number 4.

Saw gynaecologist also after MC3 who also did full clotting tests and in depth thyroid checks - I had a higher count of anti thrombin 3 so he recheck and then it was in normal range so again - deemed nothing wrong with me and I started MC4 on my follow up appointment last week.

He now wants to look at karotyping and do a lap and dye and just did a blood test for antibodies but I can't remember the name now. He freely admits he feels there is nothing wrong with me (in a trying to be positive way) and this will be all the testing that is available to us

I just don't get how 4 MCs can be normal, especially 3 of exactly the same. I can predict pretty much the day I'll miscarry and everything they are so similar.

Am I missing something?
Should there be something else I should be testing for?

I keep seeing factor v Leiden and MTHFR about but how do you get tested for those - will I already have been tested without knowing it?

Progesterone has never been mentioned. I asked last week and was told 'sometimes we look at it......' And that was it. I don't have any sort of treatment plan. He said I could have baby aspirin next time but when I told him I'd been on it for MC 3 and 4, he just didn't say anything.

I feel like as a gynae he's out of his depth. I'm considering asking for a referral to prof Quenby as Coventry is not far but don't really know if I'm wasting my time. Maybe there is nothing wrong with me.

I think you are right and your local doctor is probably out of his/her depth by now. You could ask to have progesterone prescribed from just after ovulation. After all, it is cheap and causes no harm. A lack of progesterone can cause implantation problems (uterus lining isn't thick and juicy enough), which could explain your early and consistent losses.

If you've had a full clotting panel done then that would include Factor V Leiden and MTHFR mutations.
I think getting a referral to Prof Quenby is an excellent idea! She really is a top specialist and has a few more tricks up her sleeve than your local specialist will have.

Thank you. Do they often give it out from ovulation? I was considering asking him for it next bfp but maybe I should ask for some ready to go at ovulation just in case. I wish he was a bit more forthcoming with something I could do or try but it's just so passive and 'the tests are all normal......'

It's a really hard day today. Feel like I'm going to set the house on fire with the amount of candles I'm to light tonight :cry:

LPF, you can take the progesterone from just after ovulation. The best option is to temp and start taking it once you've had a definite temp shift. You don't want to be taking it any earlier as you risk messing with ovulation. Some doctors only prescribe it from bfp but others think that if you really do have a problem then the damage is most probably done by then. I'd try and get it in advance. I was on 400mg cyclogest daily, btw.

I'm in the US and my dr said since my losses were happening consistently between 7-9.5 weeks and we saw a baby & hb, the issue was with me not DH so we've never had a sa done on him. Never did any bloodwork either. The dr said the losses were most likely hormone-related and prescribed progesterone.

Broken-sorry for your loss and especially sorry that it's dragging on like this. But the ladies on this thread are amazing. I still hop in once in a while even though I've technically 'graduated'. Have you had any testing done to see why you keep having losses?

You mentioned you graduated. Did the progesterone help? I can't make it to 5 weeks. Had 3 losses and I've never been tested for progesterone except after my last BFP and it was at .8

It *seems* like progesterone helps but only if I take a daily baby aspirin too. My hCG and progesterone levels are always on the low end of normal even if I'm supplementing but the dr doesn't feel this is an issue. All other testing (clotting disorder, thyroid, autoimmune, etc.) has come back normal so we're sticking with the progesterone/baby aspirin regimen for future pregnancies.

As for you, with such a low progesterone level at bfp (mine are low at 4+0 and they are always around 9-10. 0.8 is VERY low.) and very early miscarriages, I'm inclined to think progesterone deficiency. I'm not a doctor though so don't take this as gospel truth. But I'd def ask for a progesterone prescription the next time you get pregnant. My dr said that's the single biggest sign of hormone troubles-early miscarriages. I take the oral supplements from bfp (confirmed with a blood test) til 14 weeks when I wean off of it but some doctors will prescribe it from ovulation.

Thanks this was very helpful. I should o in the 20th and have my appointment on the 21st so I'm going to ask that they test my progesterone that day. I had a temp drop this morning so I might o a little earlier but my appointment would be less than 5 days away so I'm still in a pretty good position, I hope!
 
Aspirin, progesterone and prednisone have gotten me further with this pregnancy than the last three. I also took a load of coenzyme q10 prior to implantation.

There will be a lot of candles lit today. So sad.
 
hi ladies ive started taking progesterone after ov and wondered what sort of dose you ladies take I have the 200 mg

Kelly-For my older kids, I took 100mg of the oral progesterone twice a day from bfp til 13 weeks. With my last pregnancy, I took 200mg of the oral progesterone twice a day from bfp til 13 weeks and then weaned off it over the next week.

I didn't get to light a candle last night for my angel babies because I was at work but I said a little prayer instead.
 
I think mine is 200mg twice a day but need to check that x
 
I thought for sure mine would be more than once a day but when they checked my levels, they said just one was necessary
 
I took 400mg twice a day to 16w, I've got to stay on it longer next time, not sure how long though

That is it 400mg twice a day. Hope I stay on mine until 36 weeks x
 
the doctors wouldn't give it to me here so ive got some and been taking it anyway I think I will up the dose to twice a day if I get a bfp as ive heard its not harmful is this true ladies?
 
Ive been at loooooads of clinics (West Middlesex hospital, Queen Charlotte's Hospital, St. Mary's Hospital, Coventry Hospital and Epsom Hospital) and that is the only thing that they all agree on, it is not harmful.

Did I mention I was feeling abandoned by my hospital? I contact my old professor (from Queen Charlotte's then he went to Coventry and I followed him there) and they want to see me.
 
I can't believe no one has ever mentioned progesterone to me.....I'm going to ask next week to try some. Seriously, why do we have to do all the pushing.

Can I ask - if you take progesterone, do you get artificially darker bfps, artificially higher beta levels and symptoms etc? Or if you had these would it be a sign of a potentially viable pg?

My signs are always weak bfps, low beta, and MC 5-6 weeks - if progesterone artificially changes this but you still might MC...it's fine, I just wants be prepared really!
