TTC again with an incompetent cervix

DW and I have decided to go for kid #4. She is 36 an I 37. Kids are 9, 11, and 13. The two oldest are adopted. In our early 20s, we experienced two losses around the 20 week time frame that led to an incompetent cervix diagnosis. With an amazing doctor, DS9 was born at 32 weeks. He only spent three weeks in the NICU and is now a healthy giant.

We just decided to try again. She goes in next week to have her mirena removed. Does anyone have experience/tips on dealing with a pregnancy when you are older with an incompetent cervix? We are expecting the same type of situations to arise; early bed rest, no physical activities, possible preterm birth, etc.

We decided to try again because among other things, we have that baby urge, but also because we are in better physical, emotional, and financial shape than we have ever been before. We are better equipped to handle the situation now than we were before.
 
I was watched closet for this with number 1 but turns out my short cervix can withstand pregnancy. What my dr told me was that if they saw it shortening anymore they would put a cerclage in around 14-16 weeks. Have you discussed that with your dr? Basically it's a stitch that holds the cervix together.
